Dear Sir,

Your article was discussed recently at our journal club . As it has been a year since publication and Cassel hospital is a notable practice site in NIMHE (1)we were curious whether the unit's practice has changed given that the author's conclusions were that the stepdown model has significant advantages. If not why not,given the shorter length of stay and resultant increase in numbers treated and the reductions in self mutilation, attempted suicide and readmission.

We also wondered which of the two variables (shorter length of stay or stepdown)was considered to be the dominant factor?
